Charitable Giving

At Memorial, we understand the importance of engagement with the communities we serve and we are committed to supporting community outreach and not-for-profit agencies, especially when it comes to organizations that support health education, events and activities. The community benefit should complement the impact of our organization and fall within Memorial’s strategic initiatives.

Our community outreach is focused on, but not limited to, the following areas:
  • Access to care
  • Fitness and physical activites, fighting obesity
  • Cardiovascular disease
  • Diabetes
  • Women’s and children’s health
  • Oncology and cancer research
  • Healthcare workforce development
  • Community involvement, economic development, and community/city/chamber events
  • Senior health programs
  • Youth events and partners in education
  • Leadership programs
  • Information technology

Sponsorships that may be considered include:

  • Any organizations that support our strategic initiatives
  • Donations defined as supporting school or college sponsored academics, arts and athletics including advertising in yearbooks and athletic signage/marketing items.

To receive donations, sponsorship, or in-kind support from Memorial, please provide the following:

  • Completed Charitable Donation Form or donation letter with a brief description of project, event or activity requiring funding, amount and reason for request, event goal
  • Organization/company location - must be located in or have a chapter that serves Memorial’s defined service area
  • Mission of organization, area and populations served by the organization, and disclose association with other not-for-profits or businesses
  • Current W-9, copy of tax-exempt letter stating legal name of the organization from IRS (990) or equivalent
  • Disclose any previous funding (past two years) received from Memorial or Memorial Foundation
  • Complete list of organization’s Board of Directors and key staff
  • Address/contact person where the check should be mailed
  • Outline of reciprocal benefits, if any (ex: logo recognition, tickets, marketing, etc)
