Signing Up

How do I sign up to join the patient portal?

At your next Memorial Physician Clinic visit, provide your email address. If you choose to join myMemorialConnection, you will be sent an e-mail invitation with instructions on how to complete enrollment.

Create Your Account

  • Step 1. Watch for your email invitation to join the patient portal. If you do not receive your invitation immediately, call the Memorial Physician Clinic where you signed up or call your primary care provider’s clinic at Memorial.
  • Step 2. In the email, click on the link provided. This link will take you to a page to create your new account.
  • Step 3. You will select to create the account as the “patient” or as another person who is managing the health of the patient. This selection is important for accurate account set up.
  • Step 4. You will verify your information. You will be asked for the date of birth of the patient and the answer to the security question.
  • Step 5. Check the box to accept the Terms of Use and Privacy Policy. Select “Create An Account”. You will then be taken to a page to create a unique username and password.
  • Step 6. You can now access myMemorialConnection. For future visits to the portal, you can log in from our website by clicking myMemorialConnection link at the top of the page. You can also download the Cerner HealtheLife app for androids from Google Play or from Apple App Store for iOS users.

I don’t have an e-mail account. Can you help me set one up?

Your e-mail is private and personal to you. No one should have access to passwords or security questions needed to open a email account. It is recommended that a family member or trusted significant other assist you.

I did not receive my email invitation after I signed up. What should I do?

If you do not receive your email invitation immediately, call the Memorial Physician Clinic where your signed up or call your primary care provider’s clinic at Memorial.

I deleted my e-mail invitation. Do I have to come back to the clinic to request another?

No. We can resend the invite. All the information you gave us will remain the same.

If I want my significant other to have access, can I just forward the e-mail invite to them?

No, the invite is intended for you. If you want your spouse, adult child, or significant other to have authorized access to your account, we will send another invitation. You must also present in person to give consent for someone else to have access to your record.

IMPORTANT: You should setup access to your own account before any proxy access is given to an Authorized Representative.

I received my invitation, but I am still having trouble signing up. Can you give me tips on setting up an account?

  1. Follow the steps to create your account from an invitation. The steps are listed above.
  2. One email address is tied to one myMemorialConnection as the primary account. If the email address you used to request your invitation was already used to create another account, you will be asked to provide a different email address to establish your own patient portal account. You can share the same email address if you are only getting proxy (secondary) access to another person’s account.
  3. A parent can use one email address to establish their own primary account and use that same email address to link to the accounts of each of their children. The link to the child’s account will be a proxy (secondary) access.
  4. When a patient gives a significant other access to their account, that significant other (Authorized Representative) will receive an email invitation. The significant other (Authorized Representative) will follow the link in the email invitation and will be asked to sign in to claim that access with the username and password of the primary account holder.

Self-Enrollment Information for the Patient Portal

Can I self enroll to the patient portal?

Yes, you can self-enroll without an invitation. See all the conditions below.

  • Self-enrollment is available for patients who are age 18 years or older.
  • You can only get access to your own record through self enrollment.
  • You must have received service in our system after June 14, 2014.
  • If you have permission to access another person’s health record, you cannot set up that access through self enroll
  • If you previously created an account, you cannot re-claim access to your account through self-enrollment.

How do I access the portal after my account is set up?

You can access your account through the myMemorialConnection link at the top of every page of our website, or save the link to your desktop from the login page. If you have a mobile device, you can also download the Cerner HealtheLife app. For more information on My Memorial Connection, visit the patient portal overview page.

Can the staff show me how to login?

Sure. We are happy to help you with logging into your account. Make sure to keep your password private.

After I enroll, if I forget my username and/or password who can I contact?

Lost your password? You can reset the password yourself at the myMemorialConnection login page. From the login page, you can recover your password. Just type in your e-mail address or username in the “Username” field then click on “Forgot password?” and follow the steps as instructed.

Lost your username? You can use your e-mail address in the “Email Address or Username” field and enter the password to get into your account.

Need to update your email address? The Sign in and Security section allows you to update the email address you use when signing in, and the Notification section within the online menu allows you to update the email address where notifications are sent.

I have a medical question about the information in my account. Who should I call?

If you have a medical question or would like to speak to someone regarding the accuracy of the information in your account, please contact Medical Records (Health Information Management Dept) at 228-865-3172.

Who will have access to my records in the portal?

Only you will have access to your patient portal account. Keep your username and password secure.

You can give consent for another adult to have access to your My Memorial Connection account. This is called proxy (secondary) access. A person with proxy access is an Authorized Representative on your patient portal account.

If I request a medication renewal through the portal, how long will it take?

Usually, one to two business days.

If I request an appointment through the portal, how long will it take to get a response?

Usually one to two business days.

Can I request other records through my portal?

No, at this time, only the records that automatically show in the patient portal are available via electronic access. You may contact the Health Information Management (HIM) department at Memorial Hospital for other record requests.

Can I still get my health records from the hospital’s Health Information Management department?

Yes, you can still get copies of your medical records from Memorial’s Health Information Management (HIM) department.

How will I know if someone has viewed documents in my portal?

Your account has an access log that records when data is viewed in your patient portal account. Click on the online menu and select “Access Logs”. The location of the online menu will vary among devices. See the table of content. Only you or the person(s) you have granted access can view information in your patient portal account.

Access to Another Person’s Record

What is Proxy Access or an Authorized Representative?

You can give consent for another adult to have access to your myMemorialConnection account. This is called proxy (secondary) access. The person granted proxy access is the Authorized Representative. This Authorized Representative will receive an email invitation to access your myMemorialConnection account. They will be asked to follow the link in the email invitation and sign in to claim that access with the username and password of the primary account holder.

Can I get access to my child’s records online?

Parents or guardians of children up to the age of 18 can give consent and gain access to their child’s records online.

Can I have access to my child’s records if I no longer have custody of him or her?

As long as no court ruling exists removing your parental rights, you can have access to your minor’s (0–17 years of age) record in patient portal.

Can I block by ex-spouse from having access to our child’s records?

You must show documentation of a court ruling removing the parental rights of that parent before access can be blocked. This document will need to be evaluated before such approval is given by Health Information Management at Memorial.

I am experiencing a technical issue in myMemorialConnection. What should I do?

My provider in another town does not have the new Direct email and I am unable to send my online records to him. What should I do?

Security

Is my patient portal account secure?

The system uses an encrypted secure session between your computer and the myMemorialConnection website each time you sign into it. This encryption is enabled by using a secure socket layer (SSL) certificate, the industry standard for securing websites.

Only you or any Authorized Representative chosen by you, will have access to your patient portal account. Keep your username and password secure.

Electronic Health Record (EHR)

What are the benefits of this electronic health record?

As a patient of Memorial, your healthcare provider will have up-to-date information on you and your health. The EHR is part of a fully integrated information system that offers patients (via the patient portal) and staff one common clinical and administrative system, improved information sharing between departments, and centralized scheduling and billing. Why do I have to fill out the same paperwork at different locations?

You will have one health record when using Memorial Hospital and Physician Clinics services. This system is designed to decrease some of the repetitive paperwork. In some cases you may be asked for additional verification.

Third-Party Apps

What are third-party apps?

A third-party app is an application other than the myMemorialConnection patient portal app.

We now offer the ability for you to securely connect your health record to some of the health management apps you may use (i.e. fitness trackers, dietary trackers, etc.). This new process provides access to your health and wellness data in an app of your choosing. All third-party applications may not past the security requirements to connect to your health record at Memorial Hospital and Physician Clinics Gulfport.
