Memorial Stone County Nursing and Rehabilitation Center Receives 5-Star CMS Rating

Memorial Stone County Nursing and Rehabilitation Center has received a Five-Star rating from Center for Medicare and Medicaid Services (CMS), a rarity for continuous care retirement communities.

The Five-Star quality rating system provides residents and their families with an easy-to-understand summary of three dimensions of nursing home quality: health inspection results, staffing data, and quality measure data. The overall star rating is a composite of the three individual star rating dimensions.

“The Centers for Medicare and Medicaid Services overall star rating represents a facility’s performance level in several key areas. Memorial Stone County Nursing and Rehabilitation Center’s achievement in being named a Five-Star facility is an incredible honor, and to achieve this distinction during such a challenging time is remarkable,” said Memorial Senior Director of Senior Care Kyle Lewis, LNHA, MS, RD, LD. Lewis continued, “The achievement is a culmination of great leadership and selfless dedication among staff members to ensure the residents receive the utmost quality of care that they deserve. I couldn’t be prouder of each and every staff member that has been committed to the unwavering goal of providing the highest quality and compassionate care for their residents.”

Nursing homes with 5 stars are considered to have much above average quality. There is one overall 5-star rating for each nursing home, and a separate rating for each of the following three sources of information:

  • Health Inspections – The health inspection rating contains the 3 most recent health inspections and investigations due to complaints. This information is gathered by trained, objective inspectors who go onsite to the nursing home and follow a specific process to determine the extent to which a nursing home has met Medicaid and Medicare’s minimum quality requirements. The most recent survey findings are weighted more than the prior year.
  • Staffing – The staffing rating has information about the number of hours of care provided on average to each resident each day by nursing staff. This rating considers differences in the levels of residents’ care need in each nursing home.
  • Quality Measures (QMs) – The quality measure rating has information on 15 different physical and clinical measures for nursing home residents. The QMs offer information about how well nursing homes are caring for their residents’ physical and clinical needs.
