Immunization Criteria

To be eligible, a child should meet any one of the following:

  • has no health insurance
  • is on Medicaid, CHIP or qualified through a State Medicaid Waiver
  • is an American Indian or Alaskan Native

General health education as well as childhood immunizations and adult screenings are offered at various offsite locations throughout the community. 

Immunization Van Schedule

Memorial Immunization Van is open (unless otherwise noted):

  • Monday through Thursday
  • 8 a.m. - 12 p.m. and
    1 p.m. - 4:30 p.m.
    (Check calendar for each day as times may vary.)

Immunizations available
for children
age 18 and younger:

Parents must furnish child's shot record, social security number and Medicaid card, if the child is covered by Medicaid. All vaccines are administered by a registered nurse. There is an administrative fee for children's immunizations of $13 per dose.

  • Influenza (Seasonal)

To qualify for vaccinations, a child must meet one of the following criteria:

  • Is on Medicaid, CHIPs, or qualified through a State Medicaid Waiver
  • Is an American Indian or Alaskan Native

The administrative fee will still be charged.

aDULT SCREENINGS OFFERED

Adult screenings are offered at a cost of $5 for cholesterol screening (total cholesterol only) and $20 for Cholestech (full panel breakdown).

  • Blood Pressure
  • Glucose (for those adults who have fasted)
  • Influenza (seasonal, charges vary from year to year)
  • Cholesterol  (combined total)
  • Cholestech® (full panel breakdown)
  • Explanation of results with appropriate educational material
  • Referrals to other health care providers as needed

The van now offers Td (Tetanus), TDAP, and Hepatitis B vaccines, and Tb skin testing for adults. Payments may be made in cash, debit card or credit card. No bills larger than $20 will be accepted. Prices vary.

You will need to verify your Insurance coverage for vaccines at Memorial prior to your visit.

The Immunization Van staff will bill Private Insurance, Medicare, and Medicaid. You must bring your insurance card with you.

Flu shots 

Flu shots for adults and children are available seasonally. The cash price for flu shots is $25 per adult and $13 per child. Payments may be made in cash, debit card or credit card. No bills larger than $20 will be accepted.

You will need to verify your Insurance coverage for vaccines at Memorial prior to your visit.
